It is with mixed emotions that I announce the closing of my practice at Murfreesboro Family Care, effective August 3rd. I will be practicing full-time at the Primary Care and Hope Clinic and I invite you to continue our doctor/patient relationship by transferring your records to that organization.
Primary Care and Hope Clinic is a non-profit health center providing affordable family health services to medically uninsured, underserved, Medicare, TennCare and privately insured families in Middle Tennessee. They currently have two locations in Murfreesboro, one in Shelbyville and one in Smyrna.
I have been affiliated with the Primary Care and Hope Clinic in various roles since its inception in 1992. I believe in their mission and believe that you will be impressed with the good work that they do.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. If you plan to continue as my patient, you can sign an authorization form to release your files to the Primary Care and Hope Clinic. Use the following button to complete the form and securely submit it.
If you choose to see a different physician, please get an authorization form from my office as soon as possible so we may transfer your records to your new doctor. Until then, your records will remain on file at Murfreesboro Family Care. My office remains open to assist you with this process.
